AVALA Spine Medical Director, Dr. K. Samer Shamieh will personally review your MRI Images, and call you to discuss what he sees.

FILL OUT THE FORM

Read it carefully and make sure to answer all questions to the best of your ability. The most accurate readings should be completed within 6-12 months of the images being taken. The more recent the better. We will also need to know where you are experiencing symptoms (i.e neck, arm, hand, back, leg etc.)

MAIL THE DISC

Please send us a hard copy of the images on a physical disc. The address to send the images is:

Once the imaging arrives, our office will reach out to you to set an estimated time for a video conference with Dr. Shamieh.

ANSWER THE PHONE

Dr. Shamieh will be calling you directly. The number may look different then you are used to seeing, because of the software we use to make these calls. If you are getting a call in the agreed time set by our clinic, make sure you are ready to answer and discuss your images.

In this conversation, Dr. Shamieh will give his opinion about what he sees in the images. This is not an official diagnosis. An in-office visit is required to make a formal diagnosis. Dr. Shamieh will advise you of your next steps before the end of the conference.
